The family health and monitoring system is the basis for designing and reviewing interventions.

The family health folder was designed for collecting household level health information, covering each family. It is essentially a tool for engagement with the community that helps the community health team understand the family health situation. It is also a source of authentic data that helps the team plan and review interventions and their impact.

This folder consists of 5 cards mainly on general health, eligible couple, maternal health, child health and a follow up card. Each of these cards collects a different kind of data that helps is addressing family health as well as maternal and child health issues. The collection of data through these has been a regular activity and has played a critical role in designing field level interventions. Through these folders a total of 1450 families are covered on a regular basis.
